Helped with our hospital discharge survey? Join the briefing session

If you filled out a hospital discharge survey, you’re invited to sign up for a briefing session in which you can provide more feedback and help to shape the service.

Your feedback was greatly appreciated by the discharge team.

They will build on this with a briefing session where you can give more input how you think things can be made better.

A spokesperson for discharge team said: “As you may know, Lincolnshire County Council Adult Social Care (ASC) have staff based within hospitals in Lincolnshire as well as in out of county hospitals, who work alongside health colleagues to support your discharge.

“We wanted to know about peoples experience of hospital discharge be that as a patient, carer or provider.

“So, over the past few months HWLincs have undertaken three surveys on our behalf to gain your feedback and views.

“You came up trumps and provided us with some excellent responses that told us honestly what works well but also and perhaps more importantly the areas that we need to develop.

“We cannot thank you enough for supporting the surveys but now we have one more ask. We would like to invite you to attend a briefing session, where we would like you to advise us on how you think we, as an integrated team of health and social care staff, can make things better.”

If you are interested in attending one of these events, email Michelle Wood at or call 01476 464354.

The closing date for registering an interest is Monday, July 8th at 5pm.

Once it’s known how many people are interested in attending and where they live or work, information will be sent out with firm details.

Once again, many thanks for your help in supporting us to ensure discharge from hospital will be a good experience for everyone.
